Job Description
This healthcare organization in NYC is seeking a licensed RN to join its Community Health team in the Bronx.
- Schedule: Tuesday-Saturday or Sunday-Thursday, 8:30 am-5:00 pm, field setting with 4-5 visits daily.
- Responsibilities include conducting in-home and community assessments, managing UAS-NY assessments, developing care plans, administering medications, educating patients and caregivers, and coordinating care across settings.
- Qualifications include 1+ year of clinical nursing experience, NYS RN license, and strong communication and documentation skills.
- Preferred skills are a Bachelor's degree in Nursing and experience with frail or elderly populations (training provided).
