General Summary of Position MedStar Health is looking for a Social Worker LICSW to join our team at the MedStar House Call Program! As a Social Worker LICSW , you will screen and provide care coordination, psychosocial support to a panel of frail elders receiving home-based primary care. You will lead outreach activities within the community that align with overall MHCP initiatives. You will be a member of an interdisciplinary team of providers, including physicians, nurse practitioners, and care coordinators, responsible for ensuring outcomes aligned with care goals for your patient panel. Additionally, you will train and supervise entry-level case managers based on MHCP patient census and resource allocation. Primary Duties: Screen patients, family members, and caregivers to evaluate medical, psychosocial, financial, and environmental needs. Formulate accurate assessments, develop intervention plans, and assist with enrollment in all eligible programs. Help resolve barriers to achieve social and health goals. Maximize compliance with medical recommendations and care goals. Collaborate with the interdisciplinary team on community and government resources to support patient care. Provide education, advocacy, emotional support, and crisis intervention as needed. Utilize community resources to assist patients in resuming life in the community. Assist with applications for placements such as nursing homes, assisted living, hospice, and other treatment centers as appropriate. Document all activities, including intake assessments, care plans, communication with service providers and agencies, and all activities related to patient care administration and maintenance. Provide case management services in jurisdictions where MHCP is enrolled as a provider for 1915(c) Home & Community-Based Services Waiver. Maintain waiver service requirements, conduct annual reevaluations, and periodic reviews as per guidelines. Train and supervise entry-level case managers according to patient census and resource needs. Provide behavioral health counseling to patients, caregivers, and groups. Document and bill for these services. Participate in clinical competency reviews to improve quality of care. Qualifications: Master's degree in social work. 3-4 years' experience in the commercial ambulance business. Licensed Independent Clinical Social Worker in the respective state of employment. Current American Heart Association (AHA) certification. Home health care experience preferred.
